Geriatric Care Management
The VNA offers care for seniors who wish to age gracefully at home – or wherever they call home.
Through our Geriatric Care Management service, VNA nurses, social workers and geriatric experts help people and their families to maximize the independence, safety and quality of life of elders and others who need assistance with daily living. We know that when care is needed to remain in your home, the medical, physical, social, emotional and financial issues can be time consuming and often overwhelming to manage.
VNA Geriatric Care Management staff can help by:
- Assessing care needs
- Developing a personalized Plan of Care
- Finding the resources to provide the needed care
- Facilitating communication among family members and care providers
- Coordinating and monitoring care and the client’s needs
The VNA can help with one or all of these services. We’ll work with you to determine how we can best support you or your loved one.
