Making a Referral and Arranging VNA Care is Easy

It can be difficult to know where to turn when you or a loved one needs help with health care issues at home. The VNA is here to help.
Our Intake nurses will discuss your options, answer questions, arrange services, and clarify insurance coverage. We offer coordinated interdisciplinary services to address any home care needs, and work in partnership with referring physicians and other providers to deliver coordinated care from the simplest to the most complex cases. We'd like to introduce you to our staff of Intake & Referral Nurses.
VNA services are available 24 hours a day, seven days a week, and can be often initiated within 24 hours of the referral. Thank you for considering the VNA for your home care needs.

Nights, Weekends and Holidays:

If you prefer to leave a voicemail message for an Intake Nurse, please call 802 860-4484.
Please leave your referral request and an Intake Nurse will call you (or someone whom you designate) on the next business day to confirm acceptance of the referral. Please speak slowly and include the following information:
- Your Name,
- Telephone number where we can call you back,
- The name of the patient you are referring (please spell the name) and their date of birth,
- And, if you are calling from a physician's office or a hospital, your department or floor.
