Chronic Illness & Disease Management
The VNA offers skilled home care services to those learning to cope with the management of long-term disabilities or chronic illness. Our team of professionals works with clients, their families and their physicians in developing and providing a comprehensive plan of care to allow the client the opportunity to stay in their home setting.
No matter the client’s need, the VNA has nurses available seven days a week, 24 hours a day, with day and evening scheduled visits and an on-call service at night.
Clients may also receive:
- Education relating to health, wellness and management of the disease
- Regular assessment of the client’s current physical status and progress made
- Support in activities of daily living
- Care coordination services
- Referral to additional community services
Disease Management
The VNA offers a disease management program that provides strategies to help clients learn to control the impact of diseases such as diabetes, asthma, cancer and HIV/AIDS. Care is provided not only to help the client understand their illness but also to help them achieve the best health possible while living with their disease.
VNA Telehealth Program
The VNA's Home Care Telehealth Program monitors a client's vital signs and symptoms via a machine that transmits the information to the VNA. Telehealth monitoring has been shown to improve the delivery of care and to decrease health care costs by reducing unplanned emergency visits and hospitalizations.
How Does Telehealth Monitoring Work?
Your monitor will prompt you to check your pulse, blood pressure O2 saturation and weight daily. This data is then transmitted over telephone lines to the VNA central station computer where your nurse checks the information daily. The telehealth nurse responds to changes in vital signs and symptoms by contacting you to discuss the change. The conversation may result in further patient education, a home visit and/or treatment changes to prevent worsening of patient condition.
