Adult Home Care

The Adult Home Care program is the VNA’s largest program, providing a wide range of high-quality care in the Chittenden and Grand Isle counties for:

VNA care is coordinated by a case manager, who plans individualized care, according to each client’s medical needs. Our team includes a full range of nursing and rehabilitation specialists. In addition, our nursing assistants provide help with bathing and other activities of daily living. Our social workers and mental health nurses provide counseling and referrals to community services.

VNA team members who can help include:

  1. Nurses, including specialists in IV therapy, wounds and ostomies, mental health and rehabilitation
  2. Licensed nursing assistants
  3. Physical therapists
  4. Occupational therapists
  5. Speech therapists
  6. Medical social workers

The goal of the Adult Home Care Program is to provide skilled, Medicare-certified, state-of-the-art care to promote health and minimize pain and suffering. We work collaboratively with clients, families and physicians in developing a plan of care that will allow clients to remain safe in their home setting, to maximize independence, and to participate in community activities.

It is the mission of the Adult Home Care as well as the Visiting Nurse Association to provide care to all members of the community who need our services.
